a. Sportsmanship - "courteous with other teams, coaches and officials, shakes hands with opponents after games, shows the best IHM ideals for a student athlete"
b. Teamwork - "carrying out the coaches ideas, attendance at practice, listening to the coaches, not complaining about teammates"
c. Performance - "ability and skill at the sport played"

Note: An assistant coach should fill this out if the coach is a parent of this player.
